6.1.1. When the Licensee Self-Reports Violations

In some cases, the licensee self-reports that one or more violations occurred and may also report that corrective action has already been taken. If further investigation is not required, the licensing specialist should handle the self-reported violation in the same manner as one noted during a site visit. The licensing specialist must still document the violations on a Noncompliance Statement and Correction Plan and deliver it to the licensee as soon as possible, but no later than 10 business days from the date the self-report is made. A Noncompliance Statement and Correction Plan is completed with the notation, “Self-Reported on (date)” in the Rule/Statute Number - Noncompliance Statement column.

The licensing specialist should instruct the licensee to return the correction plan by the date indicated, even if the licensee indicates correction has already occurred. In that case, the licensee should be instructed to enter precisely what was done to correct the violation in the Correction Plan column of the form and enter “Corrected” and the date this was done in the Expected Completion Date column.

The licensee should be instructed to post the Noncompliance Statement and Correction Plan. The Noncompliance Statement will also be posted to the Regulated Child Care and YoungStar Public Search website.
