Sometimes the licensing specialist becomes aware through the media or during a monitoring visit of an event that should have been self-reported, such as the discovery during monitoring of a reportable injury recorded in the medical log book. When a licensing specialist becomes aware of an event that the licensee failed to report, the licensing specialist conducts a follow up investigation just as if the licensee had self-reported, and those results are recorded in the Complaint/Incident Reporting Module. The licensing specialist also cites the licensee for failure to report and instructs the licensee to submit a required written self-report of the incident.
If a complaint is received about an event that the licensee is required to report but did not report, or did not report within the required time frame, the licensing specialist should cite the licensee for failure to report and proceed with the complaint investigation.
