Child Care Licensing Procedure Manual
The regional staff person completes the investigation entry in WISCCRS. The regional staff person uploads all documentation of the mail investigation (DC-1, Unlicensed Child Care Complaint Response (CFS-856) and any related documents) into WISCCRS, along with the date the investigation is closed. It is not necessary to provide a narrative description of the mail investigation. When the provider has checked box 3 or 4 on the CFS-856, this date will be after the follow-up visit is concluded. In that case, the licensing specialist will document the follow-up site visit(s) made to determine whether the provider actually reduced the number of children in care as well as any enforcement action taken. If requested, the regional staff person provides feedback to the complainant and/or provider concerning the outcome of the complaint investigation. This can be done by phone or through a brief letter sent under the licensing manager’s signature.
