3.1.3 When the Returned CFS-856 is Incomplete and/or Contains Inconsistencies

If the CFS-856 is incomplete, e.g., lacking any of the requested information, the support staff may return the form or call the responder to obtain the needed information. If information is obtained by phone, this should be noted on the form with the date the information is obtained. If the CFS-856 is not signed, the form should be returned with a notation instructing the responder to complete and return the form within 5 days. If the form is not returned by this deadline or the completed CFS-856 still does not contain all required information, the complaint is assigned to a licensing specialist for an on-site investigation as soon as possible, but no later than 10 working days after the complaint is assigned.
