3.1.2 When the Provider Does Not Return the CFS-856

If the CFS-856 is not completed and returned to BECR within 7 days from the date of the DC-1 letter, the regional staff person may attempt to contact the provider by phone or mail. The provider should be told that the CFS-856 must be received within 5 working days from that contact or a licensing specialist will make an unannounced visit. If the CFS-856 is still not received after that deadline, the complaint is assigned to the appropriate licensing specialist for an on-site investigation as soon as possible, but no later than 10 working days after the complaint is assigned. (Note: Dates for receipt of the completed CFS-856 are entered and tracked through WISCCRS.)
