Child Care Licensing Procedure Manual

6.3.1 Certification of Records

Often during the course of a complaint investigation, the licensing specialist must obtain a record(s) from other agencies or offices outside the Department. When a document is obtained, the person providing it must verify that the document(s) is a true and actual copy. Some agencies or offices, especially court agencies and law enforcement, have their own certification format and this is acceptable. If, however, the licensing specialist is dealing with a person unfamiliar with certification of records, the licensing specialist should provide a copy of the standardized format for Certification of Records and explain its purpose. The standardized format is available on the BECR Intranet. The form may be faxed, mailed or hand delivered. Note: if a notary public is not available to the person completing the form, another person should sign as a witness to the signature. Also, if the document regards confidential records of a juvenile, that should be noted on the certification form.
