2.4.1 Determining Current Certification Status of Child Care Providers

To determine whether the provider is currently certified, the staff person must check the name, if known, and/or address identified in the complaint in WISCCRS. If the provider is certified, the staff person checks the box on the CFS-321 to indicate this.

 If the staff person determines that a provider is not currently certified but has a history of being certified, s/he should attach or reference documentation of the past certification to the CFS-321 and give it to the licensing specialist assigned to do the complaint investigation.
