8.4 Stipulations

Stipulations are signed agreements between the department and a licensee that set forth unique licensure conditions under which a license will be granted or continued. A stipulation may also be done in lieu of or as the result of an enforcement action. The licensee must agree to and sign the stipulation and it must be posted in a center next to the license for the term of the stipulation.

A stipulation is usually identified on the license certificate with the statement, “See Posted Stipulation dated mm/dd/yyyy”. The end date of the stipulation is the date the stipulation is no longer in effect or is withdrawn. If no end date is entered, it is presumed that the stipulation will be ongoing and in effect as long as the license is in effect and will continue to be identified on the license and the public website. At license continuation (or relocation), the licensing specialist must review and re-evaluate the stipulation to determine if it should remain in effect for the new licensing period.
