7.5 Failure to Submit All Required Application and Continuation Materials

If a licensee pays the licensing fee, but fails to submit other required application or continuation materials in the time allotted, the facility is closed (if an application) or revoked (if continuation). No refund is issued, unless, with the concurrence of the licensing manager/licensing supervisor, unique circumstances exist that justify a partial refund of the fee.
