If the probationary licensee fails to submit complete application materials and/or the licensing fee for the regular, non-expiring license 30 days prior to the probationary license expiration date, the Probationary License Expiration Warning Notice is issued, along with a Noncompliance Statement and Correction Plan (DCF-F-CFS294) indicating that this is a rule violation (for non-submittal of required materials and fees 30 days prior to the license expiration date). The warning notice is sent via regular U.S. mail and informs the licensee that BECR will verify correction of the violation when materials and fees are submitted. It also informs the licensee that failure to submit the application materials on or before the probationary license expiration date will result in expiration of the license and closure of the facility.
If the application materials and licensing fee have not been received one day after the probationary license expiration date, the facility is closed. The Closure Letter - Probationary, available on the BECR Intranet, is sent via regular U.S. mail.
