If the licensing materials and fees are not received 30 days prior to the license continuation review date, the License Continuation Reminder Letter and a Noncompliance Statement and Correction Plan (DCF-F-CFS294) indicating this is a rule violation (for non-submittal of required continuation materials and fees) is sent to the licensee via regular mail. The reminder letter informs the licensee that BECR will verify correction of the violation when materials and fees are submitted. The letter explains that if the licensee fails to submit the materials on or before the license continuation review date, late fees of $5.00 per day will be charged for up to 30 calendar days following the continuation date.
A BECR regional program associate is responsible for generating the letter and DCF-F-CFS294’s in WISCCRS and mailing them to the licensee. The program associate enters the violation data into WISCCRS with a Visit Reason of “Desk Review” and notifies the assigned licensing specialist.
The licensee must post the DCF-F-CFS294 beside the license. When the licensing specialist receives the completed application, the DCF-F-CFS294 is verified as corrected in WISCCRS.
