Child Care Background Check Procedures
To request a rehabilitation review, individuals complete the Rehabilitation Review Application (DCF-F-419), available by contacting the DCF OLC at 608-422-7041 or on the DCF website.
The requestor must indicate on the Rehabilitation Review Application form the program area(s) for which he or she is seeking rehabilitation review approval. While a person may be barred from all programs covered under Wis. Stat. ยง 48.686, or other programs under 48.685, the rehabilitation review panel will only consider whether to lift the bar for child care regulatory approval, employment or household member residency in the program(s) requested on the Rehabilitation Review Application form. If, for example, a requestor indicates he or she is seeking approval only to pursue certification as a family child care operator and approval is granted, that person cannot become a licensed child care provider without again seeking rehabilitation review specifically for child care licensure.
When the application is completed and signed, the requestor submits it with the required documentation to DCF. The requestor must provide all requested documentation within 90 days of the date the application is submitted. The rehabilitation review request is denied if the application form and any requested supporting materials are not complete within 90 days of the date of the application and the requestor does not have good cause for the failure to submit the information.
There is no time frame specified for how soon the panel must meet to consider the application after it is completed.
