2.3.1 Incomplete Background Check Request Forms

If an individual submits a paper version of the Background Check Request form with missing identifying information from Section A, the certification agency, regional licensing office or CBU staff will contact the licensee, operator or applicant in an attempt to obtain information needed in order to enter the individual in WISCCRS. If information from section A is missing on the Background Check Request form but available on the application, CBU, certification agency or regional licensing staff may enter the information in WISCCRS accordingly or return the form to the applicant, operator or licensee for completion. Information missing on the paper Background Check Request form should not be added to the form itself.

If the individual did not answer any question under section B or did not sign the form, the form must be returned to the applicant, operator or licensee for completion or signature and resubmission.

If, after the background check is completed, it is determined the individual failed to disclose a pending charge, criminal conviction, IBIS record, or CPS substantiation, the CBU will treat the Background Check Request form as incomplete or may interpret this as an intentional omission. The CBU will look at the facts of each particular case and make a decision based on the evidence presented.
