Child Care Background Check Procedures
The Background Check Request form requires the individual to disclose specific information, including (but not limited to):
Any pending misdemeanor or felony criminal charges or convictions for crimes and offenses anywhere, including in federal, state, county, local, military and tribal courts.
Adjudications by a court of law or tribal court.
Any requirement to be registered on a national, state or tribal sex offender registry.
Any current investigations or finding by a government or regulatory agency regarding abuse or neglect of a child, adult or elderly person
Any finding by a government or regulatory agency regarding misappropriated property (theft) of a child, adult or elderly person.
Any government-issued credentials or license that is not current or limited/restricted.
Residence outside of Wisconsin in the past 5 years, including county and dates.
Military discharge information.
Rehabilitation review findings.
The individual must provide complete answers to every question on the Background Check Request form. The answers provided must be truthful and accurate to avoid possible sanctions and penalties. Required information includes identifying information (all aliases, maiden name, AKAs). Dates and locations for all convictions, offenses and pending charges must be included. Background Check Request forms must be signed and dated and include the individual’s mailing address as eligibility notices will be mailed to the mailing address listed on the form.
The paper form or the electronic form in the CCPP or iChildCare may be completed and signed by someone other than the subject of the search. If the licensee/operator or other designated individual completes the form on behalf of another person, they are to sign the form under Section C.1. If the individual is completing the form for themselves, they are to sign the form under Section C.2.
If an individual submits a paper version of the old Background Information Disclosure (BID) form, the certification agency, regional licensing office or CBU staff will contact the licensee, operator or applicant and request that a Background Check Request (BCR) form is submitted instead.
The BECR regional licensing staff or the local certification agency carefully reviews the paper Background Check Request form(s) submitted with the license/certification application materials and transfers the Background Check Request form information and responses into WISCCRS using the Request Form link in the Individuals module. The paper Background Check Request form shall be scanned and uploaded in the Individuals module under Documents. The certification agency and licensing staff should also review the initial license/certification application carefully to ensure that all required Background Check Request forms have been submitted for all individuals age 10+ who are subject to the background check law, as required by Wis. Stat. § 48.686(2)(ag)1.b. The CBU uses the information entered in the WISCCRS Background Check Request forms page (Request Form link) and reviews the uploaded Background Check Request form to determine what disclosures may need to be investigated.
