2.13 Ongoing Submittal of Background Check Request Forms by Licensed and Certified Centers

The CBU will notify licensees or operators when it is time to update the Background Check Request forms required for the 5-year fingerprint background check. Providers will receive a 60-day notice and 30-day reminder by mail and in CCPP with instructions for updating their records in the CCPP.
