7 - Sanctions Due to Multiple Reasons

A revocation, denial or suspension is sometimes due to multiple reasons. For example, an operator is found to have a barred offense but there are also health and safety violations at/on the premises. The sanction notice should include all of the reasons or basis for the sanction, however, the most serious basis should be entered into WISCCRS (on decision page for denials and on category page for revocations/suspensions). For instance, a revocation due to a barred offense in accordance with the Background Check Law would be considered more serious than a revocation due to noncompliance of DCF 202.

See the WISCCRS User Guides –Changes and Creating New Providers for additional instruction.
