2.3 - Documentation of the Complaint Intake

The complaint intake details shall be documented in WISCCRS. See the WISCCRS User Guide – Complaints/Incidents for more information on entering the complaint intake. Once the complaint is entered into WISCCRS the intake form can be printed. The WISCCRS intake screen allows the certification worker to enter the following information: date, complainant source and the complainant’s name, address and phone number, if it requires immediate attention (imminent danger), indicator whether or not child was injured or medical attention needed, description of the complaint / alleged rule violation and type of complaint source.

 

If subsequent screening of the complaint intake determines that the complaint should not be investigated (e.g., the complaint does not fall within the jurisdiction of the agency or certification rules) and no further action will be taken, the complaint can be deleted from WISCCRS. If an investigation is warranted, the intake form, along with other documentation related to the complaint, may be filed in the certified operator’s file upon completion of the investigation.

 

When receiving a complaint/referral from CPS, certification workers may enter brief information in the Complaint/Incident Module, however, keep in mind information entered in the module may be viewed by others. The entry may include the eWiSACWIS Case ID number and contact information for the CPS worker making the referral. The Confidential Documents module can be used to upload a CPS report if the certification worker received a copy of the report. The complaint/incident module has fields under the “investigations” link to capture whether or not a CPS investigation occurred and the subsequent findings.
