12.1 - When an Operator Does Not Self-Report a Reportable Incident

If the certifier becomes aware of an event the operator is required to report to the certification agency but did not report, or did not report within the required time frame, the certification worker should cite the operator for failure to report and proceed with the complaint investigation. If the certification worker is made aware of an injury to a child requiring medical treatment/evaluation, but the injury was not self-reported by the operator, the certification worker should require the operator to submit a self-report utilizing the Incident Report form. Subsequent to the submission of the self-report, the injury shall be recorded using the complaint/incident module and coded as a “self-report”.
