Milwaukee Preferred Provider Registry - Application and Instructions
IMPORTANT NOTICE: The Department is continuing to accept applications on an ongoing basis from providers in Milwaukee, who are interested in becoming a Preferred Provider for the W-2 Program. Completed applications for the Preferred Provider Registry must be received by the Department of Workforce Development, Milwaukee Regional Office, 819 North 6th Street, 8th Floor, Milwaukee WI 53203. In June of 2005 the Department Workforce Development (DWD) announced its intent to establish a Preferred Provider Registry in Milwaukee. The Preferred Provider Registry is being established as part of a redesigned service delivery structure for the Milwaukee W-2 program to improve community-based services to participants, ensure greater fiscal accountability and promote workforce attachment, retention, career ladders and pathways to economic self-sufficiency.
The Preferred Provider Registry is part of a broader Milwaukee W-2 and related human service delivery system strategy to develop a continuum of services that supports W-2 participant efforts to overcome barriers to employment, create more diversity amongst the providers in Milwaukee that serve W-2 participants, maximize efficient use of W-2 resources through the utilization of existing publicly funded services, and build on the already existing Milwaukee County service delivery infrastructure where appropriate.
