The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.
As of mid September, 2009, the provider had 23 Shares authorizations for a facility licensed to serve a total of 8 children. Of those 23 authorizations, 16 authorizations (70%) were for children of employees of Vet's Learning Lab.
During an in-person visit on September 24, Department of Children and Families staff observed 3 children present but records showing several children not in attendance as being present on all other days. Of the three children in attendance, two were listed on the attendance sheet, while the third child was not among the children listed as being present.
The provider had been previously cited by a licensor on March 5, 2009 for inaccurate daily attendance records.
