Rationale for Reasonable Suspicion of Program Violation:

The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.

As of mid September, the provider had a total of 21 Shares authorizations, 17 of which were children of employees.

The provider is licensed to serve 8 children.

During an in-person visit on September 24, Department of Children and Families staff observed 3 children in attendance. The low attendance varied significantly with previous day's attendance, which showed a large number of children in attendance.

The provider has a long history of licensing violations related to attendance and capacity issues. According to attendance records, on August 19, 2009, 12 children were served between 3 and 3:30pm. On June 11, 2009, licensors could not gain entry, because there were no children or adults present.

On May 8, 2009, licensee stated that three children had started that week, but state data suggested that the children had been paid for two weeks prior. Licensor also noted that during the previous week, the facility was over capacity on three different days.
