Rationale for Reasonable Suspicion of Program Violation:
The license of the provider was revoked by the Department of Children and Families on October 16, 2008. An amended notice of revocation was sent on February 19, 2009 as a result of violations of the Wisconsin Shares program identified in an audit by the Milwaukee County Department of Health and Human Services (MCDHHS). The revocation was upheld by DHA on 9/17/09.
The provider was suspended from the Wisconsin Shares program for a six month period due to serious discrepancies in attendance and employment information provided to the Milwaukee County Department of Health and Human Services (MCDHHS) and/or other agencies by notice from MCDHHS dated February 12, 2009. The provider was also notified by the MCDHHS that, due to the inaccurate records and information, she was overpaid $56,989.79 in subsidy payments for the period of January 2008 through August 2008.
