Rationale for Reasonable Suspicion of Program Violation:
The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.
During in-person visit by investigators on September 18, two women answered the door. They indicated they were "closed" and denied the investigators access to the facility. One of the women indicated the licensee was out of town and not expected back until next week. There were no children present, which is significantly less than the 13 children referenced for the week beginning August 30, 2009. They also denied the investigators access to the attendance records.
Provider has a licensing history of repeat violations, including multiple citations that the licensee did not maintain written records of the daily attendance that includes the time of arrival and departure with birth date for each child, for duration of the time the child has been enrolled in the program.
