The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.
During an in-person visit by investigators on September 18, the father of the facility owner was present. The facility owner arrived at the facility after receiving a call from his father. There were three children at the facility, which is significantly less than the 16 children referenced during the previous payment period. There were two one week periods of attendance records missing, consisting of July 5th through July 11th, and August 29th through September 5th.
The provider has a previous licensing history of related violations. On March 13, 2009, a licensor was unable to access facility. In a subsequent licensing visit on April 15, 2009, according to center attendance records, there were 9 children in care at one time from 11:30 a.m. to 1:30 p.m. and 10 children in care from 1:30 p.m. to 2:00 p.m.
