Rationale for Reasonable Suspicion of Program Violation:
The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.
As of mid September, the provider had a total of 19 Shares authorizations, 15 of which were children of employees.
The provider is licensed to serve 8 children.
During a visit from Department of Children and Families staff on September 24, 2009, no attendance sheets were available. The staff present did not know the names of all of the children, despite the fact that 4 of 7 children belonged to the employee. The licensee was not present, and DCF staff was told that she had her records with her.
