Rationale for Reasonable Suspicion of Program Violation:
The Department of Children and Families suspended this child care provider from the Wisconsin Shares program due to a combination of red flags signifying Shares violations. These were identified through data and in person visits.
As of mid September, the provider had a total of 74 Shares authorizations, 64 of which were children of employees.
The provider is licensed to serve 38 children.
During a visit from licensors on September 24, 2009, 4 children were present. Two children belonged to the employee present. Another 12 children were reported to be on a field trip, but the employee did not know the names of the children on the field trip. There were no attendance records for the day of the visit. Some staff reported to be on the field trip had not punched in.
Previous licensing visits indicate that on multiple occasions, provider was cited for failing to maintain a written record of the daily attendance, date of birth, and the hours of arrival and departure for each child.
