Rationale for Reasonable Suspicion of Program Violation:

The Department of Children and Families suspended this provider from the Wisconsin Shares program because Milwaukee County Health and Human Services Department established an Intentional Program Violation (IPV), which had already led to the Department revoking the provider's license.

The Milwaukee County Department of Health and Human Services (MCDHHS) notified the Department of Children and Families by a letter dated January 20, 2009 that the licensee was suspended from the Wisconsin Shares program and that an overpayment recoupment was being sought. These actions were taken by the MCDHHS due to the results of an audit conducted by the county in which they determined that provider: 1) has been billing for children that were not in attendance; 2) based on the parent's work schedules, provider has been operating over maximum capacity of eight children, violating license regulations; 3) per parent's work schedules, have been operating outside regulated hours, also in violation of license regulations; 4) have sibling hours that don't match; 5) have parent's and children's hours that don't match; 6) have provided employment verification letters to the parents identified in the CARES system as employed at center and the names/hours of the employees reported to licensing and the Wisconsin Shares program do not match; 7) based on sign-in/sign-out records, have over-reported hours of attendance; 8) have some sign-in/sign-outs that don't match the authorization periods for some children; 9) have missing sign-in/sign-out records for some weeks; and 10) have duplicate sign-in/sign-out records for the same week periods with different hours logged.

Due to these serious discrepancies for the period of April 27, 2008 through January 20, 2009, the county has established overpayments made to the licensee in the operation of Little Miracles in the amount of $132,609.53.
