This numbered memo includes reporting requirements for CY 1999 for programs, activities and expenditures under the Title IV-E Incentive Funds program. The memo requires that annual reports be submitted to the Department by March 31, 2000.

This memo is to provide you with instructions to develop your report for the use of Title IV-E Incentive Funds for CY 1999. Reports for CY 1999 will be due on March 31, 2000. Attached are forms/formats that should be used to develop your report for submission to the Department. You may use replicated forms/formats for submission if that will facilitate the development of your report on a computer.

The information to be provided in your report is the following:

  1. Assurance that Title IV-E Incentive Funds were not used to supplant other federal, state or local funds for the activities identified. (Ref. s. 46.(2)(b), Stats.)

  2. An identification of the amount of funds not spent during CY 1998 and CY 1999 and the rationale for not spending those funds.

  3. A breakdown of how the funds were spent by category and by program/activity. By statute, not less than 50% of the IV-E Incentive Funds must be used for children who are at risk of abuse or neglect to prevent the need for child abuse and neglect intervention services. (Ref. s. 46.45(2)(a), Stats.) Each program/activity should include the measurable outcomes you intended to achieve and a discussion of whether or not those outcomes were achieved.

We are requesting this somewhat more detailed information in order to provide us with some baseline data in preparation for shifting to a more formalized multi-year plan. We are currently working jointly with the Wisconsin County Human Services Association (WCHSA) to develop a plan and report format for multiple years. In the near future, we will issue instructions for developing your plan and reports for the period of
January 1, 2000 through December 31, 2002.

By March 31, 2000, please submit one copy of your report to your Area Administrator and one copy to:

We have contracted with Dr. Mark Courtney at the University of Wisconsin-Madison School of Social Work to summarize the reports and issue a report to the Division by July 1, 2000. That report will be shared with all of the counties.
