 Document Summary

 Diligent and prompt reporting of completed adoptions will increase federal incentives to Wisconsin and help other children find a permanent home through adoptions. All adoptions between October 1, 1997 through September 30, 1998 shall be reported by October 31, 1998.

The federal Adoption and Safe Families Act provides incentives to states that increase the number of adoptions of children in foster care and adoptions of children with special needs. The incentives are provided only when new adoptions exceed prior levels.

To claim Wisconsin’s incentives, adoption agencies must diligently and promptly report each adoption. The Adoption Services Report (form DSL-22C) is the format for reporting these adoptions. DCFS offices and any county human/social service agency completing a foster parent or stepparent adoption enter the information into the Adoptions Module of the Human Services Reporting System (HSRS) at their offices. Private agencies and DHFS adoptions through the Bureau of Milwaukee Child Welfare will submit the Adoption Services Report on paper to:

Forms must be mailed or entered on the HSRS Adoptions Module within 30 days following the adoption to assure that Wisconsin claims for incentives accurately reflect the completed adoptions and maximize the incentive funds. Incentive funds received by Wisconsin will be used to further increase adoptions in this state.

The adoption incentives are available annually until 2002, beginning with the federal fiscal year from October 1,1997 through September 30, 1998. This incentive program helps children by encouraging prompt adoptions and is expected to reduce foster care caseloads as children find a permanent family through adoption.

Your cooperation in prompt and thorough reporting of adoptions is necessary to claim these incentives to help the children of Wisconsin.
