DOCUMENT SUMMARY

This memo accompanies the revised forms for determining the eligibility of a child in out-of-home care for federal Title IV-E funds. Also attached is the Departmentís Title IV-E Policy and questions and answers regarding 
Title IV-E.

 

A committee of county and state staff, in consultation with the Institute for Human Services Management, has recommended changes in the Title IV-E eligibility and reimbursement forms that were revised early in 1995. These changes were made to simplify the forms and to reflect changes in federal policy. The forms are the following:

DCS-201       Used in determining eligibility and reimbursability when a
                        child first enters out-of-home care.

DCS-201A     Used in conducting annual redeterminations of eligibility
                        and reimbursability.

DCS-205        Used as a worksheet in determining whether a child would
                         have been eligible for AFDC as part of the Title IV-E
                         eligibility process.

Also attached are copies of the "Title IV-E Eligibility Policy" and "Responses to IV-E Questions Posed by Wisconsin Direct Service Staff."

It is our hope that the changes in the forms and the other attachments will result in an easier process and more accurate Title IV-E claiming. We will provide training on the new forms in some format in the near future. We will notify you of those trainings as soon as they are scheduled.

The new forms, all of which have a revision date of 5/96 in the upper left-hand corner, should be used effective immediately. Please discard any versions of these forms which have a revision date earlier than 5/96.

Copies of the revised forms can be ordered by sending a completed DMS-25 Forms/Publication Requisition or other written request to: State Department of Health and Family Services, Division of

Supportive Living, BMO Communications Unit, P. O. Box 7851, Madison, WI 53707-7851. The three forms should be requested by form number. The "Title IV-E Eligibility Policy" should be requested by the title and publication number PCS-3019.
