2003 WORK PLAN Enclosed is the 2003 Independent Living Work Plan, which must be completed and submitted to the Division of Children and Family Services, Bureau of Programs and Policies by February 15, 2003. Work plans provide the Division with information about federally mandated independent living services to youths in and exiting out-of-home care and the projected budget associated with these services. If there are currently no eligible children (i.e., youths aged 15-21 years in out-of-home care placement for six months at or after the age of 15 years and/or youths who left care after the age of 17 years) at your agency, the work plan is still required. The plan will provide summarized information about the previous year and should demonstrate your agency's readiness and ability to provide independent living services to these youths when or if they enter placement. 2003 INDEPENDENT LIVING REPORTING REQUIREMENTS Enclosed are copies of the revised Independent Living Participant and 90-day Follow-Up forms. It continues to be necessary to collect and report independent living data using these forms rather than WiSACWIS. We have not received the final federal reporting requirements for this program and are unable to enhance WiSACWIS without these requirements. You will be notified when federal reporting requirements are finalized. Please begin using the revised forms January 1, 2003, and plan to submit the annual summary forms (i.e., CFS 873A and CFS 976A) containing 2003 data in February 2004. These forms continue to be available and fillable on line at http://www.dhfs.state.wi.us/forms/index.htm.
