Several months ago, you received an announcement about the availability of Title IV-E incentive funds (Numbered Memo DCFS-98-02 on Title IV-E Incentive Funding). Although there was some reference to program reporting, this memo is intended to provide additional guidelines concerning program reporting for this year as well as some direction for the next calendar year.
Specifically with regard to this year, you should submit a brief description of what
you plan to do with funds for CY 1998. The narrative should include a description of
anticipated measurable outcomes. For example, if you plan to provide a specific program to
prevent out-of-home care, the narrative should include statements of what services will be
provided and what the anticipated outcome of service provision will be, such as a
reduction in a specific number or percentage of foster home placements. In addition, the
description should include reference as to how the county will, if appropriate, link these
new services with the Family Preservation and Support governing committee. If there is a
Tribe in the county, the report should also contain a description of how you plan to work
with the Tribe to achieve the stated outcomes. The plan is due
A final report that addresses your agencys progress in achieving the plan and anticipated outcomes is due January 1, 1999. At the same time, you should also submit a plan similar to the narrative you prepared the previous year outlining the program and the measurable outcomes you anticipate addressing during CY 1999.
Staff from the Bureau of Programs and Policies and the Bureau of Community and Family Development will be available to provide technical assistance on development of the plan and the related outcomes.
