Translations and Vital Documents

This page provides information to assist our customers in their compliance with the Limited English Proficiency (LEP) requirements in the Civil Rights Compliance document.

If your agency needs guidance with obtaining translation services, please contact Earnestine Moss, LEP Coordinator, at (608) 266-5335, to obtain translation services or an interpreter from the vendor list.

  • Vital Documents requirements - A document, paper or electronic, that contains information that is critical for accessing the provider/agency services and/or benefits; letters or notices that require a response from the customer; and documents that inform customers of free language assistance.   • "Language Identification" cards
    • These cards can be printed and posted at your reception desk. Those who don't speak English can indicate what language they speak by pointing to the right card.
