The Department will conduct a complete background check every 2 years or with any submitted application on the following:
- Each licensee, including all partners in a partnership, the board president or CEO, or other designated representative of a corporation
- Each adult, non-client household resident.
Non-client household residents who are 10 through 17 years of age will need to submit a Background Disclosure Form (DHS-64) every 2 years. The Department will determine whether these individuals need the complete background check.
Licensees will need to do a complete background check within 60 days of hire and every 4 years thereafter on
- All employees
- All volunteers used to meet staff-to-child ratios
- All individuals who are contracted by the licensee to provide services to children.
DHS 12 is the rule that was developed to meet the provisions of the statute. A copy of DHS 12 and background check forms are available at the Background Checks portion of the Department's web site.
