
Suspension Of An
Adoptive Home Assessment
PURPOSE:
To ensure the appropriate suspension of any Adoptive Family Assessment process
consistent with Administrative Code HFS 51.
PROCEDURE:
- If the applicant temporarily fails to meet the eligibility criteria under HFS 51.07(8)
or the applicant is not ready to complete the adoptive family assessment process, but is
likely to remedy the circumstances within six (6) months, the assigned adoption social
worker, with supervisory approval, may suspend the application process. The decision to
suspend the adoption application must be documented on the Adoptive Family Assessment
Suspension Form, accessed from the Home Provider License window in WiSACWIS, and must be
maintained in the applicants record.
- The assigned adoption social worker must notify the applicant of the suspension decision
within five (5) working days of the approval of the adoption supervisors approval.
The decision must be provided to the applicant in writing and must include the following
information:
- The decision to suspend the adoptive family assessment process;
- The reason(s) for the decision;
- The conditions upon which the adoptive family assessment process may be resumed; and
- The process by which the applicant informs the Adoption Program of any remedied
concerns.
- If the conditions for a suspension of the adoptive family assessment process are
remedied at any time within six (6) months of the date of the written notice of the
suspension, the assigned adoption social worker must ensure that the adoptive family
assessment process is resumed immediately upon remedying the condition that led to the
suspension. A copy of the written notice must be maintained in the applicants
record.
- If the conditions for the suspension are not remedied within six (6) months of the date
of the written notice for suspension the assigned adoption social worker must ensure that
the application is closed and provide written notice of the closing to the applicant(s).
