Accepting And Assigning
Referrals Of Children
To The Adoption Program

 

PURPOSE:

To identify the procedure for accepting referrals from the county social worker following termination of parental rights and assigning the case in a timely fashion to an adoption social worker or a private partner.

 

PROCEDURE:

For Consultation Services:

  1. When the adoption social worker designated to provide consultation to the county or site becomes aware of a child for whom concurrent planning for adoption is needed they will assist the county social worker in completing the Referral for Consultation Form and submit the completed form to the Adoption Supervisor.
  2. The adoption supervisor that receives the Referral for Adoption Consultation Form shall review the form for completeness, create a case in WiSACWIS, if one does not already exist, and assign an adoption social worker to the case as consultant within two (2) working days. If the case does not already exist in WiSACWIS, the adoption supervisor will enter the case by creating a service intake. If the Referral for Adoption Consultation Form is incomplete or incorrect, the adoption supervisor will ensure the form is returned to the site or county social work supervisor with instructions for completing the form properly.
  3. Within two (2) working days of case assignment the consulting adoption social worker shall notify the ongoing or initial assessment social worker of their assignment to the case. Notification is made by sending a copy of the Referral for Adoption Consultation Form back to the ongoing or initial assessment worker with the name, location and telephone number of the assigned adoption worker in the top section of the form.

 

For Case Management Services

  1. When the adoption social worker assigned to provide consultation services to the county or site becomes aware of a child that will be TPR’d, and is in need of adoption services, the adoption social worker will assist the county social worker in completing the Intake Information Form and submit the completed form to the adoption supervisor.
  2. The adoption supervisor that receives the Intake Information Form shall review the form for completeness and create a case in WiSACWIS, if one does not already exist. The case assignment to an adoption social worker will occur within two (2) working days. If the case does not already exist in WiSACWIS, the adoption supervisor will enter the case by creating a Services Intake. If the Intake Information Form is incomplete or incorrect, the adoption supervisor will ensure the form is returned to the site or county social work supervisor with instructions for completing the form properly.
  3. Within two (2) working days of case assignment the adoption social worker shall notify the ongoing or initial assessment social worker of their assignment to the case. Notification is made by sending a copy of the Intake Information Form back to the ongoing or initial assessment social worker with the name, location and telephone number of the assigned adoption social worker in the top section of the form
