Identification Of Educational
And Supportive Resource Needs

 

PURPOSE:

To ensure the quality of potential adoptive homes by identifying and addressing educational and supportive needs.

 

PROCEDURE:

  1. All adoption social workers providing services to approved adoptive families must develop a plan to address the family’s educational or supportive service needs. The support plan must be documented on the Adoptive Family Support Plan form, accessed from the Home Provider window in WiSACWIS.
  2. The support plan may be a continuation of the Action/Support Plan developed at the conclusion of an adoptive home assessment or may be a new support plan designed to address the needs and/or concerns of an approved adoptive family.
  3. The plan must reflect the requirement that all prospective adoptive parents attend a minimum of twelve (12) hours of training per year and that all potential adoptive parents will attend the course offered through the local Technical College entitled, "Adopting A Child With Special Needs" whenever available in their area.
  4. The Adoptive Family Support Plan must include:
  • A description of the adoptive family’s placement preferences (e.g. age, sex, race, special needs of child, etc.) and the adoptive family’s strengths and resources;
  • Identification of concerns or needs within the adoptive family as indicated by the adoptive family, out of home care and/or adoption staff, and/or other references or collateral contacts;
  • Description of the outcomes (goals or objectives) to improve adoptive family functioning, knowledge and/or support system;
  • Description of services identified to address the concerns or needs including, but not limited to, community resources, support groups (Adoption Program and/or community based services), training and/or educational programs, and;
  • Established time frames for implementing the support services.
  1. All adoption social workers providing services to approved adoptive families must ensure that the caregivers within the family are informed of and encouraged to participate, as relevant and appropriate, in one or more of the resources documented on the current Education Support List.
  2. When the assigned adoption social worker becomes aware of a prospective adoptive parent’s participation in any training or support resource, the assigned adoption social worker must document the information regarding the resource, the provider, the dates of participation and the outcome on the Adoptive Family Support Plan. Any training attended by the prospective adoptive parents should also be documented on the Training Tab on the Home Provider window in WiSACWIS.
