The Department will conduct a complete background check every 4 years on the following:

  • Each licensee, including all partners in a partnership, the board president or CEO, or other designated representative of a corporation
  • Each adult, non-client household resident.

Non-client household residents who are 10 through 17 years of age will need to submit a Background Information Disclosure form every 2 years.  The Department will determine whether these individuals need the complete background check.

Licensees will need to do a complete background check within 60 days of hire and every 4 years thereafter on

  • All employees
  • All volunteers used to meet staff-to-child ratios
  • All individuals who are contracted by the licensee to provide services to children.

DHS 12 is the rule that was developed to meet the provisions of the statute.  A copy of DHS 12 and background check forms are available at the Background Checks portion of the Department's web site.
