Voluntary Paternity Acknowledgment

How to fill out the Voluntary Paternity Acknowledgment form

Complete a separate form for each baby. (For example, if you have twins, you need to fill out one form for each baby.) Use black ink and do not cross out or erase anything. If you make a mistake, start a new form.

Section I

  • Fill in each item in this section
  • Write in all names (first, middle, and last names) just as they are on the baby's birth certificate
  • Don’t forget the father's Social Security number or Individual Tax Identification number

Section II

  • You do not have to change your baby's name, you can check the NO box.
  • If you want to change your baby’s name on the birth certificate, check the YES box and write in the baby's new name on the form

Section III

Each parent must sign this form in front of a Notary Public.

  • Make sure a Notary Public signs and completes the Notary information on both the mother and father areas of this form
  • Each parent may sign the form at different times and in front of different Notaries
  • If you are under 18 years old, you must not sign this form
  • You will need to show the Notary some proof of identity

Be sure to send a check or money order * for the correct amount to file the form and the amount to get copies of the birth certificate. See the back of the Acknowledgment form for the list of fees. Make out your check or money order to Vital Records.

* Some hospitals and a few child support agencies might be able to help parents pay this fee.
