Ordering Paternity Acknowledgment Materials

Paternity Acknowledgment forms and instructional DVDs for staff and for parents are available only to authorized agencies. Child support publications are available to the public. All items are free of charge.

To ensure you have the most recent materials available, we recommend that you limit your order to an estimated six-month supply.

Voluntary Paternity Acknowledgment forms
Authorized agencies may order the Voluntary Paternity Acknowledgment, Request to Withdraw Paternity Acknowledgment, and Acknowledgment of Marital Child forms from the State Vital Records Office.

DVDs
Authorized agencies who need copies of the staff DVD or parent DVD should contact the Bureau of Child Support's Voluntary Paternity Acknowledgment Coordinator.

Child support publications
Available publications describe paternity, voluntary paternity acknowledgment, as well as, available services for mothers and fathers. Most publications are available in Spanish and Hmong.
